In Office Appointments
Your First Appointment
Please bring the following with you to your first appointment:

Insurance card - contact us in advance if you have questions about your insurance.

Prescription plan card - Bring this if your "Rx" coverage is not on your primary insurance card or on a separate plan entirely.

Medications - a complete list of medications you take.

Primary care doctor's name and phone number.
To speed the check-in-process on the day you arrive, you can fill out the forms below in advance by downloading, printing and filling them out and bringing them with you.

Returning Patients
If any of your information has changed significantly since your last visit, please download, print and fill out the Patient information and Patient History form.


Cancelation Policy
Please give us timely notice if you need to cancel or postpone an office visit or procedure, as we keep an active waiting list of patients.

For In Office Visits
We require cancellation notice 2 business days (48 business hours) in advance. A $75 fee applies for failure to comply.

For Procedures
We require cancellation notice of 1 week (5 business days) in advance. A $200 fee applies for failure to comply.
